How it works

How it works

1

1. Build a form

Your practice team builds an intake form (name, symptoms, consent, signature field — mix and match) in the dashboard.

2

2. Send the link

A link, QR code, or email is sent to the patient — no account, no login needed.

3

3. Fill out & sign

The patient fills out the form on their phone or tablet and signs digitally.

4

4. Archived & encrypted

The practice receives a signed PDF; all answers are stored AES-256 encrypted in the database.

FAQ

Frequently asked questions

Do patients need an account?

No. Every link is a single-use, random access code — no login, no app, no registration.

How secure is the health data?

All answers are stored AES-256 encrypted, every access is logged, and every access link expires automatically (default: 7 days) and is valid only once after submission.

What happens after signing?

The practice receives a signed PDF archive of the completed form, including timestamp, IP address, and a tamper-evident hash.

How do I log in as a practice?

Through the Digi Soft Hub — no separate password for MedIntake. Your MedIntake dashboard opens automatically after logging into the Hub.

Is there a trial period?

Yes — 14 days free on any paid plan, then automatic billing via Stripe or PayPal. Cancel anytime in the Hub.

Can MedIntake connect to other practice software?

Yes — via a partner API key issued in the admin panel. MedBook already uses this to automatically send an access link after every appointment booking.

Can I show my practice logo on the form?

Yes — set a logo URL and practice name under Settings in the dashboard, and they appear automatically on every patient form.
